Job Title: In-Home Services Coordinator
Position Type: Full-time, hourly, non-exempt
Hours: 40 hours/week
Location: Escanaba, MI
Pay Range: $13.58 - $17.22

Job Summary: The In-Home Services Coordinator assists with the day-to-day operations of the In Home Services program, scheduling clients as directed by the care plan and aide availability.
Qualifications:
1. High school diploma or equivalent required
2. One year of experience working with the elderly or one year of working in the human services field preferred

Requirements
• Working knowledge of computers, including Microsoft Office
• Excellent communication skills
• Strong organizational and time management skills
• Dependable transportation and willingness to travel
• Must submit to a criminal background check
DUTIES/RESPONSIBILITIES
• Schedule clients as directed by the care plan and Aide availability, making scheduling changes as necessary
• Find replacement Aides to fill the schedule when necessary
• Communicate concerns with the In-Home Services Program Manager and formulate steps to address them
• Assist with Aide supervisory visits as assigned by the Program Manager
• Manage the Axis Care database, ensuring accurate Aide and client information
• Assist with tablet management
• Prepare billing for In-Home Services
• Monitor and communicate as needed in the Vendor View System
• Maintain client confidentiality
• Be assigned to the on-call rotation
• Participate in required staff trainings and meetings
• Comply with the policies and procedures of Menominee-Delta-Schoolcraft Community Action Agency & Human Resources Authority, Inc. and their funding sources
• Perform other duties as assigned
